The Hope Action Care Inc. (HAC) was established in May 1987, originally as the
Hispanic AIDS Committee for Education and Resources (HACER).  The overall Mission
of the Agency is to provide Health Education, Health Promotion, and Support Services
to disabled, low-income, or homeless persons and their families.

Education

Hope Action Care has been doing community Health Education since 1987.  In 2003,
seven HAC community educators did street outreach to 8,199 individuals, and made
271 small group presentations to an additional 4,411 individuals.  As a result of this
educational outreach, 1,879 persons were tested for HIV with 28 new cases diagnosed
and referred to direct Medical and Social Services.

Case Management

Hope Action Care has been providing case management since 1990.  In 2003, 562
individual clients received more than 8,000 units of case management services.  These
units translate to more than 2,000 hours of one-to-one client services.

Mental Health

Hope Action Care has been providing Mental Health counseling since 1992.  In 2003,
HAC professional counselors provided 662 hours of individual and group counseling to
118 individual clients.

Substance Abuse Treatment

Hope Action Care has been providing Substance Abuse Education and counseling since
1994, and treatment since 2000.  HAC licensed chemical dependency counselors
provided 525 hours of individual and group counseling to 138 individual clients.

Food Bank

Hope Action Care has been providing food pantry since 1990.  In 2003, the HAC Food
Bank provided food to 536 individual families.  Members have visited the Food Bank
more than 11,500 times.  

Emergency & Transitional Housing

Hope Action Care has been providing Emergency & Transitional housing to homeless
and HIV positive individuals since 1992.  In 2003, 69 homeless persons received 2,797
sheltered nights and 8,344 hot meals.

Affordable Housing

Hope Action Care owns a 101-unit apartment complex.  There are 33 efficiencies, 42
one-bedroom, and 26 two-bedroom units.  Of these, 34 apartments are set aside as
affordable housing for low-income and very low-income families.
